Standardizing Hospital Pharmacy Practice in Home Hospitalization

Introduction

The growth of Home Hospitalization (HH) services demands standardized pharmaceutical care models to guarantee safe and effective medication management at home. Hospital Pharmacy (HP) departments participate unevenly in HH programs, with no clear agreement on their duties.

The goal was to create a consensus-based activity dashboard to help HP departments standardize pharmaceutical care in HH units across Spain.

Methods

A modified Delphi method was carried out in five stages:

"The expansion of Home Hospitalization (HH) services has prompted the need for standardized pharmaceutical care models to ensure safe and efficient medication management in the home setting."

This study is part of the Research Topic Pharmacists at the Frontline of Public Health: Workforce Training, Community Campaigns, and Integrated Patient Care.

Summary

A consensus-based approach was used to develop guidelines that unify hospital pharmacy practices in home hospitalization units in Spain, addressing variable involvement and unclear roles.
